When Indian athlete Santhi Soundarajan won a silver medal at the 2006 Asian Games in Doha, she became the first Tamil woman to achieve this milestone. But her triumph quickly turned to devastation. Within weeks, Soundarajan was stripped of her medal after failing a gender verification test. Her story exposes how sports organizations have used flawed science to police women’s bodies, often with devastating consequences for intersex athletes who face discrimination at the intersection of gender, class, and caste.

How gender testing began in sports
Gender verification in sports has a troubling history rooted in suspicion rather than science. The practice emerged in the 1930s when Olympic officials began questioning whether certain female athletes were “really women.” At the 1936 Berlin Olympics, rumors circulated about athletes like Stella Walsh and Helen Stephens, with accusations flying that they were men competing in women’s events to gain unfair advantage.
The International Olympic Committee formalized gender testing in the 1960s. Initially, this meant subjecting female athletes to humiliating physical examinations. At the 1966 European athletics championship, 243 women were forced to undergo what were called “nude parades” where officials inspected their genitals. These invasive procedures were eventually replaced by chromosome testing in 1968, which searched for the presence of a Y chromosome.
The justification for these tests reflected Cold War anxieties and sexist assumptions. Officials feared that Soviet and Eastern Bloc countries were sending men disguised as women to compete. This paranoia rested on the flawed belief that women could never legitimately outperform expectations without some form of deception. Notably, gender testing has never identified a single case of a man deliberately masquerading as a woman in Olympic competition.
The evolving methods of verification
Testing methods changed over decades, but each approach created new problems. The Barr body test looked for an inactive X chromosome typically present only in XX individuals. The polymerase chain reaction test searched for the SRY gene on the Y chromosome. In 2012, regulations shifted to focus on testosterone levels, requiring women with naturally high testosterone to lower their levels through medication or surgery to compete.
Each method failed to account for the natural diversity of human biology. Women with complete androgen insensitivity syndrome possess XY chromosomes but develop entirely female bodies because their cells cannot respond to testosterone. These women would be disqualified under chromosome testing despite having no competitive advantage. The International Olympic Committee suspended mandatory mass testing in 1999, though suspicion-based testing continued for individual athletes.
Santhi Soundarajan’s story of discrimination
Santhi Soundarajan grew up in extreme poverty in Kathakurichi village in Tamil Nadu. Born into a Dalit family, she lived in a small hut without electricity, running water, or a toilet. Her parents worked as brick kiln laborers, earning approximately $4 per week. Despite suffering from malnutrition as a child, Soundarajan discovered her talent for running in her early teens.
By 2006, she had won 12 international medals for India. At the Asian Games in Doha, she claimed silver in the 800 meters with a time of 2 minutes, 3.16 seconds. Then officials required her to undergo a gender verification test without proper explanation or informed consent. The test determined she did not possess “the sexual characteristics of a woman.” She was stripped of her medal and banned from competing.
The intersections of caste, class, and gender
Soundarajan’s marginalized social position amplified her suffering. As a Dalit woman from an impoverished background, she lacked the resources and social capital to fight back effectively. The discrimination she faced extended beyond the athletic federation. A colleague at the Sports Development Authority of Tamil Nadu where she later worked subjected her to years of harassment, using both casteist slurs and questioning her gender identity.
The public response was cruel. Media outlets sensationalized her story, treating her body as a spectacle for public debate. People in her community looked at her with suspicion, treating her like a cheater. The psychological toll was immense. In 2007, Soundarajan attempted suicide after consuming veterinary antibiotics. She cut her hair and changed how she dressed, desperately trying to avoid recognition.
It took years of activism, particularly from intersex rights advocate Gopi Shankar Madurai, before Soundarajan received any support. In 2016, the Tamil Nadu government appointed her as a permanent athletics coach. Yet nearly two decades later, she has never received her medal back. As she asked in a 2016 interview: “I am ready to wear churidar, to wear saree, to grow my hair long, plait it and even keep flowers in it. But can you give me back my medal?”
Why strict gender categories fail
The fundamental problem with gender testing is that it assumes all humans fit neatly into two distinct biological categories. This binary simply does not reflect reality. Approximately 1 in 1,000 people are born with some form of intersex variation, though estimates vary depending on which conditions are included.
Soundarajan has partial androgen insensitivity syndrome, a condition where cells partially resist the effects of testosterone. This can result in a range of physical presentations. Some people with this condition have underdeveloped male genitalia, while others develop female genitals and breasts. Soundarajan was assigned female at birth, raised as a girl, and lived her entire life as a woman. The presence of a Y chromosome or elevated testosterone levels does not make her male.
The double standard of biological advantage
Sports celebrate biological advantages in male athletes while punishing them in female athletes. Olympic swimmer Michael Phelps has a genetic variation that allows his body to produce half the lactic acid of typical athletes, giving him extraordinary endurance. His unusual arm span and torso length provide additional advantages. These differences are celebrated as part of what makes him exceptional.
Women with naturally high testosterone face opposite treatment. They are told their bodies provide unfair advantages and must be medically altered to compete. Yet testosterone is just one of countless biological factors that influence athletic performance. Height, lung capacity, muscle fiber composition, and metabolic efficiency all vary naturally among athletes. As sports ethicist Erin Buzuvis writes, “Saying that no one can use natural advantage is antithetical to sport.”
The focus on testosterone reveals underlying assumptions about gender. Testing protocols apply only to women’s competitions, never to men’s, because there is no perceived advantage for women competing against men. This reflects the sexist belief that women are inherently inferior athletes who need protection from those with “masculine” traits. Indian sprinter Dutee Chand, who also faced restrictions due to naturally high testosterone, put it simply: “I want to remain who I am and compete again.”
The limitations of binary thinking
Organizations like World Athletics and the International Olympic Committee have struggled to define who counts as a woman because sex determination involves multiple factors that do not always align. Chromosomal sex, gonadal sex, hormonal sex, and anatomical sex can present in various combinations. Some women have XY chromosomes. Some men have XX chromosomes. Attempting to reduce this complexity to a single test inevitably excludes people who should be included.
The current testosterone-based regulations create absurd contradictions. Non-intersex women with naturally elevated testosterone from conditions like polycystic ovary syndrome can compete without restriction. But intersex women with similar testosterone levels must suppress their hormone production. The regulations thus become not about testosterone levels but about policing which kinds of bodies are acceptable.
Critics point out that these policies cause significant harm. Athletes who were unaware they had intersex variations suddenly face invasive medical scrutiny. Some undergo unnecessary surgeries or hormone treatments with serious health consequences. The psychological damage from being publicly questioned about one’s gender can be devastating, as Soundarajan’s suicide attempt demonstrates.
