When hazardous waste is burned in the open or incinerated at low temperatures, it releases dioxins, furans, and particulate matter into the air. Communities near poorly managed waste sites often breathe in these harmful substances daily. Studies in areas contaminated by oil industry waste have documented acute symptoms including respiratory distress, headaches, and dizziness among exposed populations. In Abidjan, Cรดte d’Ivoire, approximately 500 tons of illegally dumped hazardous waste releasing high concentrations of hydrogen sulphide caused immediate health effects in nearby residents.
Ingestion through contaminated food and water
Hazardous chemicals seep from dumpsites into groundwater and soil, contaminating water supplies and agricultural products. Heavy metals, solvents, and petroleum-based chemicals infiltrate groundwater, making their way into drinking water wells and irrigation systems. When crops are grown in contaminated soil or watered with polluted sources, toxins enter the food chain. In seven Asian countries, 169 hazardous waste sites polluted by lead exposed an estimated 245,949 children under age 4, with levels sufficient to decrease intelligence quotient (IQ).
Direct skin contact with hazardous materials
People living near waste sites face risks from direct contact with contaminated soil, water, or materials. Waste workers, scavengers at dump sites, and children playing in affected areas are particularly vulnerable. Globally, more than two million medical personnel are exposed to pathogens through their work with healthcare waste. Acute health effects
Immediate exposure to hazardous waste can cause respiratory issues, skin irritation, nausea, and neurological symptoms. Research on populations near the Abidjan waste site showed significant associations between exposure and general symptoms, otolaryngological problems, respiratory distress, digestive issues, and dermatological conditions. Short-term effects may include chemical burns from disinfectants, thermal injuries from waste burning, and radiation exposure.
Chronic diseases and cancer risks
Long-term exposure to hazardous waste has been linked to various cancers. Scientific reviews have found limited evidence connecting hazardous waste exposure to liver, bladder, breast, and testis cancers, as well as non-Hodgkin lymphoma. Areas contaminated with vinyl chloride, beta-hexachlorocyclohexane, heavy metals, and dioxins show elevated cancer rates. The link between endocrine-disrupting chemicals in waste and certain cancers is particularly concerning, as these substances interfere with hormonal systems even at low concentrations.
Reproductive and developmental impacts
Hazardous waste exposure affects pregnancy outcomes and child development. Studies have documented associations with low birth weight, pre-term birth, and congenital anomalies including neural tube defects, urogenital abnormalities, and musculoskeletal problems. Unsafe injections with contaminated needles and syringes were responsible for 33,800 new HIV infections, 1.7 million hepatitis B infections, and 315,000 hepatitis C infections in 2010 in low- and middle-income countries.
Respiratory conditions
Populations near hazardous waste sites with emissions of persistent organic pollutants show increased rates of asthma and respiratory infections. Airborne particulate matter from waste burning contributes to asthma exacerbation, reduced lung function, and increased hospital admissions. The Basel Convention framework
The Basel Convention on the Control of Transboundary Movements of Hazardous Wastes and their Disposal, adopted in 1989, represents the most comprehensive global environmental agreement on hazardous waste. With 191 parties as of 2023, the treaty aims to protect human health and the environment from the adverse effects of hazardous waste generation, management, and disposal. The Convention regulates transboundary movements of toxic, poisonous, explosive, corrosive, flammable, and infectious wastes. It requires parties to minimize waste quantities transported and treat waste as close as possible to its generation point.
WHO initiatives and guidelines
The World Health Organization has developed comprehensive guidance for safe waste management. A recent WHO report warns that poorly managed solid waste is driving a public health crisis and calls for urgent action. The organization recommends promoting waste reduction, developing strong oversight systems, favoring safe treatment methods over incineration, and raising awareness about waste-related health risks. Only 61% of hospitals globally had basic healthcare waste management services in 2021, dropping to just 25% in fragile contexts.
National and regional regulations
Many countries have strengthened regulations governing hazardous waste disposal. The United States Environmental Protection Agency’s National Priority List identifies hazardous waste sites requiring cleanup, with approximately 41 million people living within a 4-mile radius of these sites. European regulations require proper identification, treatment, and disposal of contaminated sites. However, enforcement remains inconsistent, particularly in developing countries where resources and regulatory frameworks are limited.
Community-level interventions
Local action plays a crucial role in addressing hazardous waste. Effective interventions include expanding waste collection services in underserved communities, improving control at disposal facilities, eliminating open dumping and burning, and developing occupational health programs for waste workers. Education campaigns help communities understand proper disposal practices and recognize health hazards. Some regions have established specialized centers for hazardous household product collection, providing safe disposal alternatives.
Vulnerable populations at greatest risk
Certain groups face disproportionate exposure to hazardous waste and its health effects. The health sector can contribute by ensuring safe management of healthcare waste, developing occupational health programs for waste workers, and conducting surveillance to strengthen the evidence base. Most importantly, preventing waste generation at source through sustainable production and consumption patterns offers the most effective long-term solution.
